Mn health model-Public Health Interventions (The "Wheel" Manual) - Minnesota Dept. of Health

The project flips the traditional health care delivery model on its head by bringing therapy directly to people in their homes and communities rather than forcing them to bounce between hospitals, medical clinics and psychiatrists. Since then, nearly 20, Minnesotans have enrolled, with results showing early success at reducing rates of severe depression, costly hospitalizations and wait times for treatment. Yet the new model faces an uncertain future. The federal funding that made it possible is set to run out by the end of June, which could trigger staff layoffs and reduced access to care. The program has not kept pace, for example, with the burgeoning demand for home care services and a severe shortage of care workers.

In addition to strengthening clinical health care, the Minnesota model for health system transformation will emphasize community health, preventive services, behavioral health, and other support services. Minnesota will increase the kinds of care offered through ACOs, including for the first time long-term social services and behavioral health services. It will create linkages between the ACOs and Medicare, Medicaid, and commercial insurers, aligning payments to provide better care coordination, wider access to services, and improved coverage.

In , Minnesota passed health care legislation to improve affordability of health care, expand coverage and improve the overall health of Minnesotans. In addition, the Legislature mandated that the Minnesota Department of Human Services DHS develop and implement a demonstration testing alternative health care delivery systems, which includes accountable care organizations ACOs.

This led to the development of IHP, formerly called the Health Care Delivery Systems HCDS demonstration, which strives to deliver higher quality and lower cost health care through innovative approaches to care and payment. With this program, Minnesota is one of a growing number of states to implement an ACO model in its Medical Assistance Medicaid program, with the goal of improving the health of the population and of individual members.

Participating providers enter into an arrangement with DHS, by which they are held accountable for the costs and quality of care their Medicaid patients receive. Providers showing an overall savings across their population, while maintaining or improving the quality of care, receive a portion of the savings.

Providers who cost more over time may be required to pay back a portion of the losses. Beginning in , DHS expanded and enhanced this model in several important ways. DHS introduced multiple tracks to accommodate a diverse set of provider systems, added a quarterly population-based payment to support care coordination and infrastructure needs, and modified the quality measurement methodology.

We have updated this page with information on the RFP for contracts beginning in We encourage current IHP participants, health care homes and other innovative provider systems to apply.
